Forms

These forms can help you provide the personalized formation necessary to help catechumens/candidates on their journey to becoming disciples of Jesus.  They can help you manage the journey of each inquirer, from first contact to ongoing mystagogy and keep up with the required paperwork and canonical documentation for each participant.

They can be edited to meet your own parish requirements and can be used in a way that best meets your RCIA process needs – either as a sit-down one-on-one interview (always preferable), or a take-home questionnaire.

  1. Journey Checklist - To keep track of all the rites, interviews, and sacraments for each person.

  2. Inquiry Log - To be filled out by parish secretary, priest, or other ministry leader upon initial contact with a seeker.

  3. Post-Inquiry Follow-Up - For use after inquiry meetings (i.e. Alpha, Discovery series) and before Rite of Acceptance.

  4. Intake Interview - For use when an individual has expressed an interest in entering formation to become Catholic.

  5. Mid-Point Interview (Before Rite of Election) - For individuals who have participated in the RCIA meetings, have indicated a desire to proceed to Initiation, and have been seen to be ready for Initiation at Easter by RCIA leadership and the Pastor.

  6. Mystagogy Interview (A few weeks after Easter) - This interview should be scheduled well in advance, even before Easter, in order to ensure follow-up with both the neophyte and the godparent.
